A chronic medical condition is one that is likely to be, or has already been present for 6 months or more. Although there is no list of eligible conditions, those with cancer, asthma, diabetes, heart disease, stroke, or musculoskeletal conditions generally qualify for chronic disease management services. Chronic disease management plans are designed for patients who require a structured approach to their ongoing care. CDM plans help identify an individual's health care needs, outline the services to be provided by the general practitioner and health care professionals, and determine the actions the patient needs to take.
A patient with a chronic medical condition may be eligible for services under a Team Care Arrangement (TCA) or a General Practitioner Management Plan (GPMP). Because these plans help GPs coordinate your care, your need for ad-hoc consultations should reduce.
If your chronic disease management plan includes visits to associated health providers, such as a physiotherapist, you may be eligible to get Medicare benefits for these services. Medicare is Australia’s universal health care system; as such, we offer Medicare chronic disease management plans for our patients where possible. We also accept Medicare insurance, so if you're in receipt of this, we are able to assist with your chronic management as well as acute conditions. Our practice also accepts insurance from most of the major medical providers.
