Chronic Disease Management
Chronic disease management plans are a key part of our commitment to the health and well-being of our community. Our doctors provide tailored management plans for patients living with chronic medical conditions, ensuring comprehensive and ongoing care.
Chronic Disease Management
Chronic Medical Conditions
A chronic medical condition is one that has been present for at least six months or longer, such as:
- Asthma
- Cancer
- Cardiovascular disease
- Diabetes
- Musculoskeletal conditions
- Stroke
Chronic Disease Management Plans
These plans are designed by general practitioners for patients with complex conditions requiring ongoing care. A GP Management Plan
(GPMP) provides an organised approach to care, helping you manage your condition effectively. Your GP will work with you to:
- Identify your health and care needs
- Set out the services to be provided
- List the actions you can take to help manage your condition
Regular Reviews
Once a plan is in place, it’s important to have regular reviews with your GP. This ensures your goals are being met and allows for any necessary adjustments to your care plan.
Our GPs specialise in managing chronic health conditions such as high blood pressure, diabetes, high cholesterol, asthma, emphysema, arthritis, osteoporosis, and long-term back pain. If you have a chronic health problem, we encourage you to speak with our GPs, who have a special interest in helping patients with chronic diseases.
